Having ownership and responsibility of your health and care will enable you to keep as well as possible and help prevent serious illness – allowing you to make the best out of life!
Self-care packs for Long Term Conditions
Bradford Districts CCG have developed packs for five of the most common conditions for self-care in the Bradford area:
- Stop Smoking
- Reduce Alcohol Intake
- Maintain a Healthy and Well-Balanced Diet
- Stay Active and Exercise regularly
- Learn basic first aid and keep a well stocked medicine cabinet
